Sexuality and Television Essay After viewing only a few hours of television I asked myself, How is it that people can spend as many as 20 hours watching television per week? A whole day wasted, and what has come out of it? More garbage that will further distort any real comprehension held about what it means to be a male or a female. What it means to be male or female in the American culture cannot be defined with simple words, but yet can be easily distorted when viewing any commercial on television. It is amazing how a story about an entire cultures views about sexuality can be told in only a small portion of the day. This story about sexuality was unraveled as I forced myself to watch something I have absolutely no interest in and have not seen in almost four months. Women wearing extremely revealing clothing (i.e halter tops, mini skirts, breast accentuating shirts, and translucent clothing), men inconsequentially fooling around with whom ever they please, and negatives attitudes about genders were all common issues in my 19 inch box of colorful sound. Could it really be possible that the programs on television accurately portray how we Americans view sexuality? Watching only five hours of television led me to the conclusion that no matter what channel I happened to turn on, there was always an attractive person being displayed, and Americans choose to be promiscuous because they can. Viewing such an abundance of attractive people on television makes me wonder if a normal American even exists. If our lives do not resemble those of the characters we view on television, does that mean that there is something wrong with us? In the American society, if we see something that we like, we will keep watching it and obsessing over it until the trend is exhausted, even if this trend is a human being. According to Byrnes law of attraction, our liking for a person is influenced by the reinforcements we receive from interaction with them. This also says that shared activities provide the basis for sm ooth and rewarding interaction. In other words, the more time a person spends with someone doing things that they mutually enjoy (shared activities), the more attracted they will be to each other. Byrnes law is completely contradicted when it is applied to the notion that many adolescent girls fall madly in love with stars such as the members of Backstreet Boys. There are no interactions between the obsessive girls and the superstars, but yet these girls believe that they are destined to be with the group members. There are never shared activities with these stars, but yet the ones who feel an unexplainable bond with their destined one cry when beautiful women are seen around their man. One can only assume that attractiveness is all that matters in the America, and if you dont have attractiveness, you are hopeless. Although music videos contain a variety of musical styles, they have many common visual themes that inaccurately portray famous artists. These themes are the degradation of women, and the promiscuity of men. Many of videos display women as sexual objects to be looked at while they rhythmically dance to the beat of the song. Aspects of pain assessment in individuals, who are sedated on ventilation support, and nursing interventions to identify levels of pain will be critically analysed and justified. Finally, the complexity of patient’s care will be argued and critically evaluated and the wider context of care management will be conscientiously discussed. The International Association for the Study of Pain (IASP) outlines the definition of pain as an â€Å"unpleasant sensory and emotional experience associated with actual or potential tissue damage, or described in terms of such damage† (IASP, 2017). In accordance to Chanques et al (2007) pain is one of the mo st occurring complications associated with intensive care stay with an occurrence of moderate to severe level of pain experienced by almost 50 % of medical and surgical patients. Bertollini et al (2002) reported that approximately 80 % of patients on critical care units, amongst Europe, have experienced pain during nursing interventions and have not received adequate analgesia. In multicentre studies 50 – 65% of patients complained that they suffered severe pain in ICU; 15% were unhappy with the pain management they received. To maintain confidentiality and to protect the identity of an individual, the name of the patient will be changed to Mr Kenneth Smith (pseudonym), in line with Nursing and Midwifery Council (NMC,2015). Kenneth is an 74 year old male admitted to cardiac intensive care unit (day 10) following coronary artery bypass graft surgery. Kenneth was previously diagnosed with triple vessel coronary artery disease, following myocardial infarction four weeks prior to surgery. Currently, he cannot maintain his own breathing, therefore, to promote haemodynamic stability Ken is ventilated with a use of synchronised intermittent mandatory ventilation (SIMV) mode. To facilitate Ken’s ventilation, he is also sedated however, it has been found that Ken reacts to voice and occasionally opens his eyes. The possibility of delirium also emerged as the patient’s wife deems, that Kenneth appears disorientated and agitated at times (Kings compromising successful post-operative recovery (Rakel and Herr, 2004).   Twycross (2002) argues that pain appears to be a problem in acute settings, yet the knowledge and understanding of its management is limited amongst nursing staff; which breaches the principles of effective practice in line with the best available evidence (NMC, 2015). Awareness of causes and interactions of pain, agitation and delirium in Kenneth’s case is essential, to promote quicker recovery by managing pain effect ively and weaning patient off the ventilation support (Stein-Parbury and Mckinley, 2000). Endotracheal tube combined with sternal wound and chest drains contributes to nociceptor activation and sensitization at various levels (Dahl and Kehlet, 2006). Hence, Kenneth is experiencing ongoing pain at rest, that increases with movements. Although, pain analgesia is administered on regular basis to control discomfort levels (WHO, 2015); Koppert et al (2003) argues that increased pain sensitivity in surgical patients to stimuli is commonly recognised as an effect of opioids. The thesis is strongly supported by Wilder Smith (2006) who acknowledge, that hyperalgesia has a major effect on the way brain processes pain signals. There several mediators that contributes to nociceptor activation: prostaglandins, interleukins, cytokine and neutrophils (Brennan, 2017). Peripheral neutrophilic granulocytes (NG’s) also have an impact on peripheral sensitivity and contribute to pain where the in cision was made (Carreira et al, 2013). The intensity of pain is amplified by nociceptive neurones that send electrical signals to central nervous system along axons, which again depends on many factors, including surgical site and discomfort caused by endotracheal tube or Kenneth position (Steeds, 2016). Recent Kenneth’s arterial blood gas reading revealed, decreased tissue pH and increased lactate concentration, which in accordance to Brennan (2011) could also influence the intensity of pain and spontaneous pain behaviour. Initially, Kenneth was commenced on mechanical ventilation post operatively to maintain his haemodynamic stability, decrease cardiac workload and provide optimum for gaseous exchange (Goldsworthy and Graham, 2014). Upon physician assessment, Kenneth receives SIMV, which delivers set amount of breath and encourages patient to take spontaneous breaths (Wiesen et al, 2013). SIMV mode helps to reduce Kenneth’s cardiac output, although it has been p roved that it prolongs ventilation dependency. Due to persistent episodes of hypoxemia, weaning from ventilation is delayed, which put Kenneth at risk of further complications (Chlan, 2011). Pillbeam and Cairo (2006) conducted a study which shown that prolonged ventilation increases the mortality rate. Challenges faced by nursing staff in pain management influences the process of weaning, hence it affects Kenneth’s quality of life (Baazm et al, 2014). Nelson et al, 2010 argues that patients who struggle to wean of ventilation support are at greater risk of infection, delirium, pain and they are often experiencing anxiety. Therefore, it could be questioned whether, unmanaged pain is a leading cause of prolonged ventilation or vice versa. Whitehouse et al (2014) reports that pain can trigger stress response which can have detrimental effect on patient’s health and wellbeing. Increased levels of catecholamines can cause arteriolar vasoconstriction, impair tissue perfusion and decrease tissue oxygen partial pressure which would impact on delayed weaning of ventilation support and increased oxygen consumption (Barr et al 2013). Catabolic disturbances and hypoxemia caused by pain can also impair wound healing process and lead to wound infection; pain decreases natural killer cell activity, cytotoxic T cell and neutrophils which are pivotal for immune system (Woolf, 1989). Wagner et al 1998 emphasises that other factors such as sleep disturbances and anxiety can aggravate pain perceptions. Royal College of Nursing (2003) concludes that nurses have duty of care to minimise distress and suffering from pain. The â€Å"gold standard† in pain assessment by nurses is self-reporting by the patient, providing necessary information, that helps to identify source, intensity and underlying condition so that appropriate analgesia can be administered (Chanques and Constantin, 2010). but Kenneth cannot give sufficient response due to endotracheal tube and se dation level (Craig and Schlavenato, 2010). Pain is difficult to measure as it is an individual experience and everyone perceive it on their own way. It encompasses wide spectrum of physiological and psychological responses; therefore, it is pivotal for nurses to provide holistic approach when looking after critically ill patients (Woodrow and Moore, 2004). Kenneth during morning activities, whilst being assisted with personal hygiene needs appeared to be in discomfort, especially during moving and handling manoeuvres. Bird (2003) argues that there is no specific pain assessment tool for ICU patients and should evaluate effectiveness of tools used, to manage pain effectively. Chanques and colleagues (2010) have manifested that visually enlarged numeric rating scale (NRS) from 0 (minimal discomfort) to 10 severe pain, is the most successful and valid across other pain assessment tools available. Jensen (2003) has reviewed 164 journals and demonstrated that NRS tool is the most reliab le indicator of pain intensity. Although Kenneth can blink his eyes occasionally when asked about pain levels, it is difficult to determine the accuracy of patient’s perception due to level of sedation and cognition state. Kenneth’s effort to express and communicate pain is respected and adequate pain analgesia is provided. The evidence shows that the belief of patient’ report creates trust relationship between patient and healthcare professional, therefore pain management is more successful and effective (D’Arcy, 2011). Pain can increase with activity and it is revealed as dynamic process, therefore Kenneth is reassessed on a regular basis to maintain comfort and minimise distress (Dahl and Kehlet, 2006). Nurses derived the total BPS score of 8-10, which indicated that Kenneth was experiencing severe pain on assessment. D’arcy (2011) speculates that the use BPS is not as effective and developed as current tools that have been in use for years. Li and colleagues (2008) also argue that there is some evidence about BPS, an updated revision in regards to validity and effectiveness is needed, so that it can be applicable for ICU patients. Its effectiveness was a gain reported during moving and handling manoeuvres, where ventilation machine was observed for alarms, which displayed that Kenneth stopped breathing spontaneously. Media Analysis: American Horror Story American Horror Story is an FX horror-drama television show, as well as an anthology series; each season of the show has a different cast and storyline. The show was created by Ryan Murphy (creator of the FOX show Glee) and Brad Falchuk (producer of Nip/Tuck), and premiered on October 5, 2011. The first season of the show follows the Harmon family as they settle into their new home in Los Angeles. They are unaware, however, that the mansion is haunted by its many former owners. The two main topics in which we covered in class that are utilized in the show are frightening and sexual content in media.The focus of the first season is on infidelity and temptation. Ben and Vivien Harmon moved to Los Angeles from Boston with their daughter Violet in an attempt to start over and save their fragile marriage, after Ben had an affair with one of his students. Although they think the move will help, it only worsens their situation. The cable series is full of as much violence and sexual content as the writers and producers can get away with. With multiple instances of violence, sex, and nudity in every episode, the show draws a very large audience.American Horror Story is FX’s most viewed series, with the pilot of the first season bringing in 3. 2 million viewers. The show gained viewers as it progressed, and the premiere of the second season had 3. 85 million viewers. You want people to be a little bit off balance afterwards. You want people to have their friends sleep over that night.And you want to deliver iconic images that stay with people. † The writers of American Horror Story make use of many of the subjects from chapter 13 of our textbook. There are many realistic scenarios that occur in th e series, such as a home invasion and multiple murders. This is an example of stimulus generalization. Because many of the scenes are highly realistic, the generalization stimulus is very high, which consequently increases the fearful or emotional response. American Horror Story is rated TV-MA, which means that it is intended for mature audiences, and not children (usually age 17 or older).It also has a sub rating of LSV (offensive language, strong sexual content, and violence and gore). R-rated videos and magazines contain far more profane and explicit sexual content than network television shows, but sexual remarks and suggestions are becoming ever more frequent in public media today. She tries to get him to sleep with her but he goes to another room and masturbates as well. These scenes, along with many others throughout the season, go back to the theme of infidelity and temptation. The trailer for the first season of American Horror Story gives the basic plot of the show. It also makes use of frightening music and sounds to get the attention of fans of the horror genre.The season two trailer also highlights each character’s traits and occupations briefly (Leo’s photography, Shelley’s sensuality, Lana Winters’ love for her partner, Wendy, etc. ). These short advertisements appeal to viewers by the use of catharsis. The audience of American Horror Story wants to be scared. It’s a way for them to escape, or animate, their own violent predispositions or inclinations; to purge themselves of their personal worries and apprehensions. Personally speaking, I find the sadistic and erotic nature of the show enjoyable.It allows me and other viewers to gain vicious pleasure by identifying with the immoral and shady personalities of the characters in the show. The intent of the show can be pretty well summed up by the theories of scholar and professor, Dolf Zillmann, in Fundamentals of Media Effects (Bryant and Thompson): â€Å"Zillmann (1991a, 1991b) described horror as frightening because it releases empathetic responses toward victims and makes viewers apprehensive about becoming victims themselves. In other words, viewers identify with the victims and experience their terror vicariously.Horror also frightens viewers because of their apprehensions; they fear being victims themselves. Finally, horror usually features a satisfying ending that viewers enjoy. † American Horror Story has had its share of controversy with viewers already. Erin Brown, contributor for the Culture and Media Institute at the Media Research Center, writes: â€Å"The premiere of â€Å"American Horror Story† wasn't just sexually and physically repulsive. Flashback scenes also featured a large dose of verbal and mental cruelty toward a Down syndrome girl – including her mother, played by Jessica Lange, wishing she'd aborted her.To add to the violence, sex and abuse, there were 13 versions of the word â€Å"s**t,† and such delightful terms as â€Å"p***y† and â€Å"c**ksucker. † With all this objectionable content, Entertainment Weekly still named American Horror Story one of its â€Å"Top Ten Things We Love this Week† putting it on their famous â€Å"Must List† in the October 7 issue. â€Å"This show has a potential to literally be shattering to all of the things that we consider to be normal ,† one of the actors said during production. † Many parental reviews of the show describe it as disturbing, terrifying, and even repulsive.The three main age groups discussed in the Reactions to Disturbing or Frightening Media Content chapter of Fundamentals of Media Effects, 3-8, 9-12, and 12-17, are all thought to be too young for most parents. Some critics also consider American Horror Story to be a strained and overexcited mess. Though there are many frightening elements to the show, most of them are written off as cliche. The fact that in society today we are so used to seeing violence and sexual content in media, these happenings in the show are not as disconcerting as they would have been in the decades prior to the twenty-first century.
